RCA AND FMEA

Introduction:

Healthcare organizations accredited by the Joint Commission are required to conduct a root cause analysis (RCA) in response to any sentinel event, such as the one described in the scenario attached below. Once the cause is identified and a plan of action established, it is useful to conduct a failure mode and effects analysis (FMEA) to reduce the likelihood that a process would fail. As a member of the healthcare team in the hospital described in this scenario, you have been selected as a member of the team investigating the incident.

Scenario:

You can find the scenario attached below in the attachment section.

Requirements:

Your submission must be your original work. No more than a combined total of 30% of the submission and no more than a 10% match to any one individual source can be directly quoted or closely paraphrased from sources, even if cited correctly. Use the Turnitin Originality Report available in Taskstream as a guide for this measure of originality.

You must use the rubric to direct the creation of your submission because it provides detailed criteria that will be used to evaluate your work. Each requirement below may be evaluated by more than one rubric aspect. The rubric aspect titles may contain hyperlinks to relevant portions of the course.

A.  Explain the general purpose of conducting a root cause analysis (RCA).

1.  Explain each of the six steps used to conduct an RCA, as defined by IHI.

2.  Apply the RCA process to the scenario to describe the causative and contributing factors that led to the sentinel event outcome.

B.  Propose a process improvement plan that would decrease the likelihood of a reoccurrence of the scenario outcome.

1.  Discuss how each phase of Lewin’s change theory on the human side of change could be applied to the proposed improvement plan.

C.  Explain the general purpose of the failure mode and effects analysis (FMEA) process.

1.  Describe the steps of the FMEA process as defined by IHI.

2.  Complete the attached FMEA table by appropriately applying the scales of severity, occurrence, and detection to the process improvement plan proposed in part B.

Note: You are not expected to carry out the full FMEA.

D.  Explain how you would test the interventions from the process improvement plan from part B to improve care.

E.  Explain how a professional nurse can competently demonstrate leadership in each of the following areas:

•  promoting quality care

•  improving patient outcomes

•  influencing quality improvement activities

1.  Discuss how the involvement of the professional nurse in the RCA and FMEA processes demonstrates leadership qualities.

F.  Acknowledge sources, using in-text citations and references, for content that is quoted, paraphrased, or summarized.

G.  Demonstrate professional communication in the content and presentation of your submission.

Patient Education Technology Guide To A Mobile Health Application PowerPoint

Purpose

The purpose of this assignment is to demonstrate the skills of the professional nurse as an educator. You are to prepare a guide that will serve as a handout to assist a specific patient that you identify. Your guide or handout should help this patient find and evaluate a reliable mobile health, or Health application (app) that is already developed. This may be related to the patient’s disease process or diagnosis, or may be an app that can help a patient maintain or improve good health, and prevent illness.

You will develop the guide using Microsoft PowerPoint. PowerPoint is a versatile application that lets you design slideshows and handouts. For this assignment, you will be using PowerPoint to create a guide or handout that you may print and give to patients and families. You will be submitting this PowerPoint file for grading.

Course Outcomes

This assignment enables the student to meet the following Course Outcomes.

CO1: Describe patient-care technologies as appropriate to address the needs of a diverse patient population. (PO1)

CO5: Identify patient care technologies, information systems, and communication devices that support safe nursing practice. (PO5)

Directions

  • You are required to complete this assignment using the productivity tools required by Chamberlain University, which is Microsoft Office 2013 (or later version), or Windows and Office 2011 (or later version) for MAC. You must save the file in the “.pptx” format. You are required to cite your source(s) as it relates to your application slide. Other citations are permitted, but this is not a requirement for the assignment.
  • Title slide (first slide): Include a title slide with your name and the title of the presentation.
  • Scenario Slide (one slide): This slide should include a brief scenario, then identify a patient who is experiencing a specific disease process or diagnosis who would benefit from an already developed and reliable mHealth app. Or it could identify a person who is currently healthy and would like to maintain or improve health and prevent illness. Be sure to include the nurse’s assessment of the patient’s learning needs and readiness to learn. Be specific.

Example:

Scenario for Ms. Ellis

Jane Doe (your name here)

  • Jennifer Ellis, a 62-year-old African American woman, has been recently diagnosed with chronic kidney disease (CKD). She has been prescribed several medications she must take every day.
  • The nephrologist has stressed the importance of leading a healthy lifestyle to slow or stop the progression of CKD.
  • She is interested in ways in which she can better track her health and make healthier choices.
  • She is a high school graduate and iPhone user, mostly to send text messages to family and friends.
  • She is eager to learn how to use an app that can help her manage her CKD.

Prepare the following slides as if you are presenting them to the patient.

  • mHealth application slide (one to three slides): Identify a developed and reliable mHealth app that could benefit the patient. Describe the app, including the following.
  • Name
  • Purpose
  • Intended audience
  • Mobile device(s) upon which it will operate
  • Where to download or obtain it (include a working link if it is to be downloaded from a website)
  • Any other information you believe would be pertinent to this situation
  • Be sure to cite all sources you use.
  • Teaching slides (one to three slides): Prepare slides that contain important points about the app that you want to teach to the patient, such as how to use the app safely and effectively (including how to interpret and act on the information that is provided).
  • Evaluation slide (one to three slides): Describe how you would determine the success of the patient’s use of this app. For example, include ways to evaluate the effectiveness of the teaching plan that are a good fit for the type of app or focus on specific ways that this app benefits the patient’s health and wellness.
  • References (last slide): List any references for sources that were used or cited in the presentation.

Writing and design: There should be no spelling or grammatical errors. Writing is concise and clear. Avoid words that the patient may not understand. Slides are visually appealing, incorporating graphics, photographs, colors, and themes.

Review the section on Academic Integrity Policy found in the RNBSN Policies. All work must be original (in your own words) unless properly cited.

nursing Paper

Assignment 1: Diversity Business

Due Week 3 and worth 130 points

You are the manager of a new primary care clinic located about twenty five (25) miles outside of a small city (population of 50,000). With five (5) family physicians, two (2) nurse practitioners, two (2) physicians’ assistants (PAs), and twenty (20) clinical support staff consisting of RNs, LPNs, and CMAs, the clinic provides primary care services to a diverse community of people living and working outside the city limits. Originally a rural area, the community has been growing and now includes promising opportunities in employment, education, and comfortable living spaces for young families. However, there are still many residents who struggle to make ends meet with older farms that have belonged to families for generations.

The central city includes two (2) large acute care facilities, and one (1) tertiary care facility that is known for its excellent pulmonary care. Both acute care hospitals provide the usual services such as labor and delivery, outpatient surgery, chronic diseases care, etc. and have fully equipped ancillary departments, such as lab and radiology. Up until this point, the residents have used the facilities’ emergency departments for routine illnesses and conditions when their private physicians were not readily available.

Write a five to seven (5-7) page paper in which you:

  1. Analyze some of the key social, political, and economic factors that have led to the proliferation of urgent care facilities and primary care practices over the last 20-30 years.
  2. Create a comprehensive mission statement for the clinic, and discuss how it will facilitate the provision of quality services.
  3. Analyze and discuss one (1) or more directions the clinic might take to grow its business. Determine what factors you would consider when deciding what services to provide in-house and which ones to affiliate with other institutions.
  4. Decide how you will determine if the clinic is meeting its goals. Identify three (3) performance measurements you could use to evaluate the success of the clinic’s services. Begin by naming a goal, and then identify a quantifiable measurement you could use for each to determine if you are coming close or falling short of the goal.
  5. Determine how you would then address whatever opportunities for improvement seem to exist and what processes you would put in place.
  6. Use at least three (3) quality resources in this assignment. Note: Wikipedia and similar Websites do not qualify as quality resources.
  7. Format your assignment according to the following formatting requirements:
    1. Include a cover page containing the title of the assignment, the student’s name, the professor’s name, the course title, and the date. The cover page is not included in the required page length.
    2. Include a source list. Citations and sources must follow SWS format. The source list is not included in the required page length.

The specific course learning outcomes associated with this assignment are:

  • Analyze the critical management issues, purpose, functions, and performance measures of different departments within healthcare organizations.
  • Examine the political, economic, and social forces that have influenced the organizational foundations of the United States healthcare system.
  • Explain how public policy has shaped the development of the U.S. healthcare system.
  • Use technology and information resources to research issues in health services organization management.
  • Write clearly and concisely about health services organization management using proper writing mechanics.
